Thank you for scheduling an appointment with Valley Women’s Health. We ask that you please download and fill out the necessary forms and arrive 20 minutes early with your insurance card/cards and photo ID in hand. Printing and filling out these forms will help us make your first appointment as pleasant as possible.

If you have an extensive list of medications you are currently taking please bring the list with you to the appointment. If you have any question or concerns regarding any of this information please feel free to call our office.

Our office participates in most insurance plans that serve the Yamhill County area, including specific plans offered by the companies listed below. If your insurance company is not on this list, please call our office to make sure we are in network with your plan. 

It’s a good idea to verify participation, referral requirements and benefits with your insurance carrier by contacting them directly. We will bill your insurance company for the cost of your visit and any additional services such as laboratory and radiology. Please bring your health insurance card to each visit. When checking in for your visit, please inform the receptionist of any changes in your address, contact preferences, and/or insurance coverage.
